Health Certificate - Immunization Record

In order to participate in the Worsham College Embalming Laboratory and Anatomy Laboratory, it is necessary for each student to submit a completed Certificate_of_Health

The form must be completed and signed by a physician. 

The following required immunizations are:

• D.P.T Series
• Booster
• T.B. Patch Test
• Hepatitis Vaccination - 3 Shot Series

Once complete, please deliver to the school registrar.
